Limitations & cautions

Free text varies a lot. Short forms and misspelled names can slip past once in a while. Always check the flags before you export, above all on handwritten notes that OCR turned into text, where letter errors are common.

How is this different from full Safe Harbor de-identification?

Redaction pulls the PHI you pick to meet the Privacy Rule's minimum-necessary standard. Safe Harbor is the stricter list of all 18 IDs for a formal claim. Use Safe Harbor when the data must fall outside HIPAA.

Will redaction hurt the clinical meaning?

No. IDs go, but the diagnosis, drugs, doses, and plan stay. The note is still useful for review.
